Learning Auto Insurance (Non-Region Specific) -- Newbie Questions..

These questions are generalized enough that i feel they are not region-specific so all answers are welcome...

I decided to write my questions inside an HTML file so you could better see a tabled example, to help you better answer my questions....so please click this link to understand my questions better (the questions are mentioned below the table too):

1. Is Jack insured to to use the GMC Tundra without asking for permission from Jill, who is the "Primary" for that vehicle, despite the fact that he is not recorded as a "Secondary" or an "Occasional" driver for that vehicle?

2. For every driver mentioned in this policy (in this case, Jack and Jill) does that mean that s/he is immediately insured for EVERY vehicle mentioned therein, regardless of driver assignments, or lack thereof?

3. There'll be a set premium to cover both vehicles in what is specifically listed in the tables above. But will the premium charge go UP in price if say Jack adds his name as "Secondary" to the GMC Tundra (as you can see, Jill is the primary on that car)?
